Under the agreement, tens of thousands of Anthem policyholders and their Community physicians will have access to health management resources from newly-formed Primaria Health LLC.
Community Chief Executive Officer Bryan Mills says the contract is "an important step on the path to a value-based system focusing on preventative health and wellness efforts. The agreement will involve about 70,000 Anthem policyholders. The health insurer says, under the partnership, Community and independent physicians will proactively reach out to Anthem members to help them manage chronic conditions and improve their health.
Community Health, in partnership with Chicago-based VillageMD, announced the formation of Primaria Health earlier this month. At the time, Mills called the current fee-for-service health care model "not sustainable," and said the new joint venture would improve patient outcomes and lower health care costs. The new company partners with Community and independent primary care physicians in 38 central Indiana counties.
